Triangle Tilt and Humeral Surgery: Meta-Analysis of Efficacy and Functional Outcome

For individuals with OBPI (obstetric brachial plexus injury) looking for an increase in active abduction and external rotation, which surgery provides the best result? The Humeral Osteotomy or the Triangle Tilt?

We searched through 731 articles and found 9 studies reporting Mallet Scale results for the Humeral Osteotomy and 4 studies that reported Mallet Scale results for the Triangle Tilt.

Here is the comparison of the Mallet scale results:

We used modified total Mallet functional score in this analysis. Various studies with humeral surgery showed improvement of 1.4, 2.3, 5.0, and 5.6 total Mallet score, whereas triangle tilt surgery showed improvement of 5.0, 5.5, 6.0 and 6.2.

We were able to conclude that when it comes to the management of shoulder function in patients with obstetric brachial plexus injuries, the Triangle Tilt provided a greater increase of active abduction and external rotation which in effect greatly increased the overall function of the arm.
